You can book appointments online and request repeat medication via an APP

If you wish to join this new service, please download the Patient Access APP from Apple or Google Play Stores. Then fill in the application form, which you can download or print off from this link https://bit.ly/3OOqkde once filled in, please return it via email to lidomedicalpractice@lmp.gpnet.je

If you do not want to send it via email, you can always post it or drop it at reception.

When we have received your form, we will email you a link code to enable you to link your App to our surgery.
