Privacy notice – registering as a new patient

This notice explains what Lido Medical Practice does with the information you give us when you register as a patient. Publish it on the website and link to it from the registration form and the practice's main privacy notice.
Who we are. Lido Medical Practice is the data controller for your registration information. - What we collect.
Your name, date of birth, gender, address and contact details; next of kin; details of children you are registering; employer and insurer, where relevant; your previous GP; a health questionnaire covering your medical history, allergies, medications, family history, smoking, alcohol and screening history; and copies of your passport or other photo ID, a recent proof of address and your Social Security health card.

- Why we collect it.
To register you with the practice, open your medical record, confirm your identity and Jersey residence, contact you about appointments, request your records from your previous GP where you ask us to, and to understand how many patients register each month and how they found us.

- Our legal basis.
Under the Data Protection (Jersey) Law 2018 we process your registration details because they are necessary to provide you with GP services and to verify who you are. We process your health information because it is necessary for medical purposes and is handled by health professionals and staff bound by a duty of confidentiality. We do not rely on your consent, so you cannot withdraw it, but you can exercise the rights below.

- Information about other people.
If you give us details of a next of kin, please let them know you have done so and that this notice applies. Children under 18 should be registered by a parent or guardian.

- Who processes it for us.
The online form is provided by Zoho Corporation and hosted in Zoho's European data centres. Registration tracking uses Google Workspace. Both companies act only on our instructions under a written data processing agreement. Your medical record is held in EMIS, our clinical system. We do not sell your information or share it with anyone for marketing.

- Where it is stored.
Your form submission is stored in the European Union. Your medical record is held in Jersey and the United Kingdom.

- How long we keep it.
Your form submission, including uploaded documents, is deleted from the form system within 30 days of being entered into your medical record. Copies of identity documents are deleted once we have verified them. Your medical record is kept in line with our medical records retention schedule.

- Your rights.
You can ask for a copy of the information we hold about you, ask us to correct it, ask us to delete or restrict it where the law allows, and object to certain processing. Contact lidomedicalpractice@lmp.gpnet.je to exercise any of these. You can also complain to the Jersey Office of the Information Commissioner at jerseyoic.org.
