If you are interested in joining our practice as a new patient, please select and complete the appropriate form. We will contact you via email within 14 days to confirm whether your registration has been accepted or declined.

Please note that our patient list is constantly changing, and we cannot always guarantee capacity. We aim to keep the list small enough to offer same-day appointments and ensure continuity of care by allowing you to see the same GP at each visit. We believe this personal, ongoing relationship is something our patients highly value.

How to Register

COMPLETE THE FORM

Please complete the appropriate registration form. Clicking on the appropriate image above will open up the registration form.

You will need a computer, tablet or mobile phone to complete.

DOCUMENTS NEEDED

In order to complete the registration form you will need the following:

Passport or driving licence
Proof of address, e.g. utility bill, bank statement, no older than 3 months
Your Green Health Card. If new to Jersey please use your Blue registration card

CONFIRMATION EMAIL

Once you have completed the electronic form, please press the submit button and it will be automatically sent to the surgery for review.

You will receive a Welcome email with further information about our practice.

CONFIRM YOUR REGISTRATION

Once your form has been reviewed by a member of our team, you will be notified if you have been accepted or declined as a new patient.
